File system verify or repair failed – Info Mac

Recently i’ve been getting some issues with my Mid 2012 13″ MBP where I’d get hangs in loading just about any program. Beach ball of death for a few seconds up to a minute or two. Finally I ran a firstaid on the disk and it failed and eventually I had to reboot at which point it would make it about 75% into loading and then turn off. After loading into Recover mode I’d also get failure to fix with Disk Utility. I’ve run it several times now and it’s always the same result. I can’t even mount the drive to access the files with a portable boot drive running 10.11.4

Any Apple Genius out there willing to lend a support hand?
